Final prepublish hygiene for autoresearch-mcp v0.4.0.

  • Remove the last fake-score eval template fallback and add regression coverage.
  • Add package whitelist guard and verify packed tarball smoke locally.
  • Update CHANGELOG for PR #3/final hygiene.
  • Archive completed v0.3.0 OpenSpec and clarify v0.4.0 close criteria.
  • Intentionally land AGENTS.md observability standard and local worktree ignore block.

Local gates: bun test (110 pass), typecheck, lint, build, npm pack dry-run audit, bun pm pack installed-tarball smoke.

Tracked by bead autoresearch-mcp-41t. npm publish remains blocked on interactive auth in autoresearch-mcp-7qa.
